RESUMO

Root proximity is defined as a situation where the distance between the roots of adjacent teeth on radiographs is ≤1.0 mm. This important situation should be detected by clinicians before definitive restorative treatments and they should be well aware of different approaches available for the management of this situation. The purpose of this study was to collect and review the available literature on this topic by searching the PubMed, Google Scholar, Scopus, Embase, and Cochrane Library in order to summarize the complications and treatment plans for root proximity in cases requiring restorative procedures.

Objectives: Endocrown restorations were introduced for endodontically treated teeth as a conservative treatment. However, data about the effect of preparation design on marginal integrity and fracture resistance of endocrowns are lacking. The purpose of this systematic review was to investigate the effect of preparation design of endocrown restorations on marginal integrity and fracture resistance. Materials and Methods: Based on PICO question and the search terms, PubMed, Embase, Scopus, and the Cochrane Library were searched. After including studies matched to predefined inclusion and exclusion criteria, the extracted data were tabulated in a table provided by the authors. Two reviewers assessed the methodological quality of each included study independently. Ten articles were selected for extracting the quantitative data. All included studies were in vitro. The potential risk of bias of the selected studies was assessed using the modified MINORS scale. Results: Four studies assessed the marginal adaptation, five studies evaluated the fracture resistance and just one investigated both the marginal integrity and fatigue resistance of the specimens. The evaluated influencing items in preparation design were as follows: cavity depth, occlusal thickness, ferrule effect, internal divergence angle, type of finish line, and adding vents inside pulp chamber. Meta-analysis could not be done due to heterogeneity of preparation designs and evaluation methods. Conclusion : Marginal discrepancy of endocrowns is intensified with adding preparation features, higher cavity depth and increasing the divergence. Fracture resistance of endocrowns is increased with more occlusal reduction and cavity depth. However, it is still beyond the normal clinical force range.

ABSTRACT Objective To assess the effects of coloring beverages on the color stability of two types of hybrid ceramics with different surface treatments. Material and Methods 180 specimens of two hybrid ceramics (Vita Enamic and Mazic Duro) and a feldspathic ceramic (Vita Mark II) were prepared (n=60 in each group). Half of the discs in each group were glazed while the other was polished. The specimens were then divided into three subgroups and immersed in distilled water, carrot juice, and coffee. The overall color difference (∆E) was calculated based on CIE L*a*b* color space. Data were analyzed using three-way and one-way ANOVA; Tukey's honest significant difference was also done for pairwise comparisons (α=0.05). Results Vita Mark II specimens revealed less overall color changes compared to other groups. The ∆E of the glazed Vita Enamic specimens was greater than polished specimens following immersion in distilled water (p=0.03) and coffee (p=0.001), but it was not significant for carrot juice. The same results were obtained for polished Mazic Duro specimens. Relatively similar amounts of ∆E were recorded in polished and glazed subgroups of Vita Mark II. Conclusion The ∆E of hybrid ceramics was higher than Vita Mark II. Polishing could be recommended for surface treatment of hybrid ceramics instead of glazing, saving time and facilitating the process.

PURPOSE: Implant mechanical complications, including screw loosening, can influence dental implant success. It has been shown that torque values are affected by contamination occurred in implant-abutment (I/A) interface. This study aimed to examine the effects of blood, saliva, fluoride and chlorhexidine contamination on reverse torque values (RTVs) of abutment screws in oral conditions. MATERIALS AND METHODS: 50 fixtures were mounted into the stainless-steel holders and divided into five groups (n = 10). Except control group (NC), fixture screw holes in other groups were contaminated with chlorhexidine (CG), saliva (SG), blood (BG), or fluoride (FG). Abutment screws were tightened with a digital torque meter. I/A assemblies were subjected to thermocycling and cyclic loading. The mean RTVs were recorded and data were analyzed with one-way ANOVA and Tukey test. RESULTS: Except for specimens in SG (20.56 ± 1.33), other specimens in BG (21.11 ± 1.54), CG (22.89 ± 1.1) and FG (24.00 ± 1.12) displayed significantly higher RTVs compared to NC (19.00 ± 1.87). The highest RTVs were detected in CG and FG. CONCLUSION: The obtained data robustly suggest that RTVs were significantly affected by fluid contaminations. Specimens in FG and CG displayed the highest RTVs. Therefore, clinicians should have enough knowledge about probable contaminations in I/A interface in order to manage them during clinical procedure and to inform patients about using oral care products.

Zirconia has been used for rehabilitation of edentulous spaces approximately for a decade, and there have been several reports regarding the clinical performance and retention of zirconia crowns. Outstanding mechanical properties, biocompatibility, and excellent aesthetics make zirconia-based crowns as a popular crown among the current all-ceramic crowns in restorative dentistry. However, restoration with a zirconia crown is a challenging treatment. The goal of this study was to assess the current literature to summarize the studies reporting the effective risk factors on retention of zirconia crowns to provide clinicians with a useful point of view in the decision-making process for use of these restorations. Literature based-search was performed to find related articles until August 2020 using EMBASE, Google Scholar, and MEDLINE. Search terms used were "zirconia restorations properties," "zirconia crowns clinical performance," "zirconia crown survival," "biological complications," and "zirconia crown retention." Results were limited to papers available in English. The references of all related literature were also searched for further citations. Overall, although clinical long-term and follow-up studies are a vital requirement to conclude that zirconia has great reliability, it seems that zirconia crown restorations are both well tolerated and sufficiently resistant.

BACKGROUND: High percentage of biomechanical complications such as screw loosening in dental implants can be related to implant-abutment (I/A) connection properties which affect the behavior of implant assembly against functional loads in the oral cavity. The aims of the present study were to compare the reverse torque values (RTVs) and failure loads of three abutment types with internal Morse taper connection. MATERIALS AND METHODS: In this experimental in vitro study, eighteen implants (4.5 mm × 10 mm bone level implants, Implantium, Dentium Co, Seoul, South Korea) were divided into three groups with different abutments: two-piece (TP) abutment, one-piece (OP) abutment, and screw abutment (SA), mounted in stainless-steel blocks according to ISO 14801. After completion the torque/detorque protocol, a compressive load (1 mm/min) was applied at 30° off-axis until failure. Mean reverse torque/tightening torque (RT/TT) values and failure loads were analyzed with one-way ANOVA test and Tukey's honest significant difference (α = 0.05). Failure modes were evaluated by radiography and stereomicroscopy. RESULTS: RT/TT values in the TP group were lower than those in other groups (P < 0.001). Highest failure loads were observed in SA group (P = 0.002). In radiographic evaluation, all specimens showed deformation in I/A interface. CONCLUSION: I/A connections with larger surface areas may lead to higher RTVs (e.g., OP and SA groups). Use of an additional screw and indexed area in TP group did not reduce the bending resistance under static loading.

OBJECTIVES: The aim of this study was to investigate the stress distribution of different post and core materials in radicular dentin by three-dimensional finite element analysis (3D FEA). MATERIALS AND METHODS: Twelve 3D models of a maxillary central incisor were simulated in the ANSYS 5.4 software program. The models were divided into three groups; the first group included: 1-Gold post and core and 2-Nickel-chromium (Ni-Cr) post and core restored with metal-ceramic restorations (MCRs). The second group included: 1-Stainless steel post, 2-Titanium post, 3-Carbon fiber post, 4-Glass fiber post, and 5-Quartz fiber post with composite cores and MCRs. The third group included: 1-Zirconia post and core, 2-Zirconia post, 3-Carbon fiber post, 4-Glass fiber post, and 5-Quartz fiber post; the last four models had composite cores restored with all-ceramic restorations (ACRs). Each specimen was subjected to a compressive load at a 45-degree angle relative to its longitudinal axis at a constant intensity of 100 N. The models were analyzed with regard to the stress distribution in dentin. RESULTS: Two stress concentration sites were detected in the models. The first group showed the lowest stress levels in the cervical region, while the stress levels detected in the second group were higher than those in the first group and lower than those found in the third group. Fiber-reinforced posts induced a higher stress concentration between the middle and cervical thirds of the root compared to other posts. CONCLUSIONS: According to the results, since cast posts induce lower stresses in dentin, they are recommended for clinical use. Fiber-reinforced posts and ACRs caused the maximum stresses in dentin.

RESUMO

In teeth with furcation involvement, root amputation is one of the treatment choices. A challenge which a dentist may encounter with is the prosthetic treatment of such teeth when their adjacent teeth are intact. According to the current goal of operative dentistry based on conservative treatment, it would be desirable to do in a manner resulting in minimal damage to the adjacent sound teeth. In the following case report, a step-by-step conservative treatment sequences of a mandibular molar with distal root amputation which not involving surrounding teeth is described. During 18 months follow-up, the results were satisfactory.

RESUMO

Treatment of patients with microstomia due to scleroderma is complicated. Limited mouth opening and altered finger shape present difficulties at every step of the prosthetic rehabilitation. This article describes the prosthetic management of an edentulous patient with severe microstomia induced by scleroderma. From among the existing treatment options and according to the patient's ability and financial considerations, the authors provided a simple prosthetic design that effectively facilitated the patient's rehabilitation. To plan treatment for a patient with scleroderma, it is important to have knowledge about existing complications, alternative methods, and the patient's ability and comfort.
